Projected Graduation Date Release

Instructions for completion: Please complete the following form in its entirety. Please include the candidate name, the last 6 of the candidate’s SSN, and the projected graduation date. To add additional releases, click on the plus sign to the right of the candidate information.

Submit this PROJECTED GRADUATION DATE RELEASE to the Conference to authorize candidates to sit for the National Board Exam (NBE) within 45 days of their official graduation. NBE results will not be released until the Conference has received VERIFICATION OF GRADUATION.
